02/24/00 MRI LUMBAR SPINE WITHOUT CONTRAST

CLINICAL HISTORY: Low back pain and decrease left leg pain.

TECHNIQUE: The following pulse sequences were performed: sagittal T1W, sagittal T2W, axial
T1 and T2W covering disc spaces T12-Ll down through level L5-S1.

FINDINGS: The lumbar vertebral bodies are normal in height and demonstrate normal alignment.

At level L1-2 of the posterior disc margin, intervertebral foramina and facet joints are within
normal limits.

At level L2-3, minor bulge of the posterior disc margin is present, but minor indentation on the
dural sac. Intervertebral foramina and facet joints are within normal limits.

At level L3-4, mild bulge of the posterior disc margin is present with mild indentation on the dural
sac. Intervertebral foramina and facet joints are within normal limits.

At level L4-5, moderate loss of disc space height is present with mild bulging of the posterior disc
margin, and mild indentation on the dural sac. Osteophyte formation is noted posterolaterally.
Mild encroachment upon the inferior aspect of the intervertebral foramina is noted bilaterally,
more on the left than on the right. A mild degree of spinal stenosis is present at this level.

At level L5-S1, moderate degenerative disc disease is present with loss of disc space height,
desiccation of the disc space. Mild bulging of the posterior disc margin is present with mild
indentation on the dural sac. Mild hypertrophy of the facet joints is noted. The iigamentum flavum
is prominent. A mild degree of spinal stenosis is present at this level. Moderate encroachment
upon the left L5-Si intervertebral foramina is present, and mild encroachment upon the right
foramina is noted.

The S1 vertebral body is partially iumbarized.

IMPRESSION:

1. DEGENERATIVE DISC DISEASE IS PRESENT AT LEVELS L4-5 AND L5-S1
TO A MODERATE DEGREE WITH BULGING OF THE POSTERIOR DISC MARGINS.
MILD DEGENERATIVE DISC DISEASE IS PRESENT AT LEVEL L3-4.

2. A MILD DEGREE OF SPINAL STENOSIS IS PRESENT AT LEVEL L4-5 AND
LEVEL L5-S1 DUE TO THE COMBINATION OF BULGING DISC MARGINS, FACET
JOINT PROMINENCE, AND HYPERTROPHY OF THE LIGAMENT FLAVUM.

3. ENCROACHMENT UPON THE LEFT L5-S1 INTERVERTEBRAL FORAMINA OF A
MODERATE DEGREE IS NOTED.
